  • My library card is for personal use. I am responsible for all use of this card.
  • I will be charged for items I do not return (based on average replacement costs).
  • If I lose my library card there will be a $2.00 fee to replace it.
  • The library may suspend or limit service if I have fines or billed materials.
  • I will report any name, address, telephone, or e-mail address changes to keep my record current.
